How to revise code templates?

I have just made a new code template and forgot to add an item.
How do I revise/modify/delete an existing code template?

PS: I do not get the situation when the "Last Modification Time" works, how did it suppose to work?

You can add, delete or modify code templates
via menu "Tools > Options... > [-] Syntax settings > 'your syntax' > Code templates"

Last Modification Time is time when current file was last modified. 
So, let say, after every save of the file and new entering of the template, you should get new values.
